After performing this task, an individual filling out this form will need to make an appearance in a government facility that can complete the application process.

One such office that falls under that government department is a local post office. When going to one of these offices, individuals should make sure they have all the documentation necessary to complete the transaction.

With Form DS-11, an individual will need to provide evidence of U.S. citizenship, an original copy of primary identification, a copy of that primary identification, two copies of one photograph, and the financial means to pay for the fees.
